CC and Associates Service Descriptions
1. Community Engagement:
Fosters the ability of the individual to acquire, retain, or improve skills necessary for behavior, interpersonal competence, greater independence and personal choice in typical activities and function of community life such as those chose by individuals without disabilities. This service provides a wide variety of opportunities to facilitate and build relationships and natural supports in the community while utilizing the community as a learning environment. These activities are conducted at naturally occurring times and in a variety of natural setting in which the individual actively interacts with persons without disabilities (other than those paid to support the individual).
Group Day Support (provided in a community setting)
2. Group Day Support (provided in a community setting}:
Skill building or supports for the acquisition, retention or improvement of self-help, socialization, community integration and adaptive skills. Day Support provides opportunities for peer interactions, community integration and enhancement of social networks. Supports may be provided to ensure an individual's health and safety. Services occur in a non-residential setting, separate from the individual's home. Services are normally furnished two or more hours per day on a regularly scheduled basis for at least two or more days per week. This service is designed to enhance self-care and hygiene, eating and toileting, task learning, community resource utilization, environmental and behavioral skills, social skills, medication management and transportation skills.
Community Coaching
3. Community Coaching:
Services which are designed for individuals who need one to one support in a variety of community settings in order to build a specific skill or set of skills to address a particular barrier or barriers preventing an individual from participating in activities of community engagement.
Residential Supports
4. Residential Supports:
a. In- home support services: The provision of community support services and other structured services to assist individuals to strengthen individual skills and that provide environmental supports necessary to attain and sustain independent community residential livings. Services include drop in support to more intensive supports, monitoring and training. Services are primarily provided with a 1:1 DSP ratio but may occur with a ratio of 1:3. Services occur in the individual's home, or community settings and typically supplement the primary care giver.
b. Group home with 4< individuals: Services are provided in a home operated by CC and Associates and licensed by the Department of Behavioral Health and Developmental Services. DSP supervision and monitoring are daily and available on a 24-hour basis AND with other individuals receiving residential support simultaneously. These services allow for skill-building, routine supports, general support and safety supports that allow an individual to acquire, retain, or improve the self-help, socialization, and adaptive skills necessary to reside successfully in home and community based settings.
c. Independent Living Supports: A residential service provided that offers skill building and support to secure a self-sustaining, independent living situation in the community and/or may provide the support necessary to maintain those skills. Supports may be provided in the individual's residence or in other community settings.
d. Supported Living Services: Residential support services that are provided in a licensed setting and enable the individuals to have access to around the clock support via a timely response when needed and support to develop skills needed for daily life. The allowable activities include but are not limited to: using community resources, personal care activities, developing friends and having positive relationships, building skills, daily activities in the home and community and support to be healthy and safe.
